Travel Waiver *

I am giving my child permission to ride the By Your Side vans or busses to and from day camp and to and from day camp events such as but not limited to the recreation center, movie theatre, splash pad, and any other prearranged location. I will not hold Bayside Church or By Your Side Family Hope Mission Liable for any potential damages that may incur.

Swim Waiver *

I give By Your Side Family Hope Mission permission to take my child to swim at locations such as but not limited to the local pool, splash pad, beach or any other prearranged locations. I will not hold Bayside Church or By Your Side Family Hope Mission liable for any potential damages that may incur.

Damages Waiver *

I agree to pay for any damages that are incurred by my child outside of the normal wear and tear of the Bayside Church and By Your Side Family Hope Mission facilities.

Liability Waiver *

I agree that neither By Your Side Family Hope Mission or Bayside Church are to be held liable for any injuries that may happen to my child or children while at By Your Side Family Hope Missions Summer Day Camp.
